The National Committee for Quality Assurance (NCQA) has awarded KDMS Portsmouth Family and several other King’s Daughters centers as excellent Patient-Centered Medical Homes (PCMH). PCMH is a model for care. The patients are at the forefront.
“PCMH recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said Lisa Ashley, King’s Daughter’s primary care team leader.
An intent to build better relationships between patients and clinical care teams is the basis of the form. The award signifies that primary care facilities, are doing just that.
This recognition shows the commitment and dedication to providing an improvement in quality within their practice. A patient-centered approach to care.
The NCQA uses evidence-based findings to award their marks of distinction. The organization is a private, nonprofit dedicated to improving healthcare quality.
NCQA recognized nearly 20 King’s Daughters locations as outstanding Patient-Centered Medical Homes. Portsmouth Family Medicine was one of them.
